What does 谁在作怪 mean?

According to my dictionary, 作怪 means 'do mischief; make trouble', so 谁在作怪 should mean 'Who is making trouble?'. However, this doesn't really fit the context, and one of my colleagues told me it means 'What is the reason?', so I would just like confirmation of what it actually means.

Originally, it means, "Who is making the trouble?"

Now, it could mean "what is the problem" or "what is the reason".

The rhetorical question is more like the following:

(1) What's going on?

(2) Who's behind the scene [causing this anomaly]?
