And write at the side (in English) Name of recipient, Leshan City, Sichuan Province, PROC, China.

If you want to put more of it into "English", use Pinyin and you get the following (in reverse order):

I never saw PROC for the People's Republic. It's usually PRC or just China. For the rest, it's just as Hanlink says.

P.R. China is what I always use when mailing stuff. If you just put China, there's a chance it may end up in the ROC.
