Purchase Approval

Hey looking for a bit of help just starting with how to translate the word purchase approval (as in the form that people fill out). I've got a friend saying 商品认可 but it feels a bit narrow in terms of all that the form typically can encompass.

Then from there things like requester name, description of items, approver, requisition number, etc. If I can find a good name I'm sure google will help with some of the rest of it。

The goverment has a system of control over the purchase of some potentially dangeous items, say, poisonous chemicals.

Ok, this is just an internal document for a company. It holds NO legal significance. It's just so that the 财务部 knows that the purchase of say office supplies, or a computer was approved by the guy who does the budget.
