Health Certificate?

I am attending BLCU in September, and need to apply for my X visa... When I apply for it, I have to have the Physical Examination Certificate filled out...

However, the letter from BLCU states "You must present the original health certificate and blood test results issued by a hospital in your country to the Beijing Health and Quarantine Bureau to obtain the Health Certificate"

Is this the same form I am getting filled out for the visa, or a different one?

I will to use the same form that I used when I applied for the scholarship. This form's title is "physical examination record for foreigner". And this form can be downloaded from the website of the confucius institute or the chinese embassies. I have found the form after a very quick search on the net.

Hey I'm actually studying at Tsinghua this upcoming school year, so I needed an X visa as well. My admission notice said that I would need to turn in the health form for the visa, but I haven't had the form completed yet and I was issued an X visa without it. You only really need the health form for the residence permit.

That's interesting, I think it depends on the embassies in different countries and the policies they follow. But it is very useful to know that we will need the original health certificate with us when appliying for the residence permit.

Interesting that they are asking for the original one.

I did the medical tests when applying for the scholarship and sent the original (didn't think of making a copy) to the ambassy. I hope they send it back with the admission package... I guess I can always go back to the clinic and ask for a copy (I know they kept one, but it's not the "original").

I didn't get the original copy back with the admission package. I already had the copies, but the embassy here does not accept them, they said they only accept the original ones, so I am doing all the medical process again :(

I think it really depends on the embassy. In my case, the embassy actually told me "I suggest you do those tests in China when applying for the residence permit", and applied for the X visa without them.

Ah, hopefully this embassy doesn't care too much about it either. I submitted it with my application, but the doctor that filled it out was a total quack and didn't want to give it the time of day... his handwriting was horrible (hello, Chinese people will be reading it!) and he wasn't very complete with it. I had to ask him multiple times to fill out different sections, and he left it with that I have COPD and a heart disease, which were just incorrect readings due to the fact that I'm skinny and tall... :(
