What would be an appropriate (but preferably fairly literal) translation of the bold part in the quote above? I'm just wondering if the 在里面 part refers to the cause of his condition, and what the 还 in 他应该还有 means. Is it refering to tuberculosis and diabetes, or is refering to something in addition to those things?

I have taken a look at the original question. I think 在裏面means in his body. 他應該還有 is used to mean that there is something inside that can't be felt or seen outside, because the wife says the husband does not feel unwell at all. Diabetes and TB are just examples here.

The causes of emaciation are many, for example, tuberculosis and diabetes. (Besides this (unknowable from the original)), (I suppose) there might be other underlying causes (of his emaciation). You should take note.

还有 is used to suggest that there might be other factors involved.

在里面, as I see, is tantamount to ‘within his emaciation’ or better ‘of his emaciation’.
