Repair Invoice/Receipt in Traditional Chinese and English

Hello,

I am currently studying in Taiwan and my computer needs to be fixed.  It is covered by a warranty but the warranty company will not accept an invoice in Chinese.  

 

I will need to bring an invoice in Chinese and English to the repair shop so they can fill it out.  

 

Would someone be able to translate this invoice form into Traditional Chinese, so that both the repair shop and the warranty company can read the form?

 

The words I need are:

BILL TO: Customer  Name Address   City, ZIP Code Country Contact Phone DATE: INVOICE #: Sales Rep. Name     Model Repair Description Quantity Unit Price  Total

I would really appreciate the help as I will be stuck without it!
