Desperately seeking advice - Chinese birth certificate

My future spouse is in the same situation. It would be very useful if someone wrote what's the Chinese name of "Birth certificate issued by notary" or explain in Chinese, that he "has to go to the notary office serving his birthplace". Also, what if he was born at home and no hospital recorded it? He has a Hukou though.
