I'm not sure what corrected vision refers to but on my form it says "N/A" (not applicable), even though the doctor wrote "wears reading glasses" in the "Eyes" section next to it. Perhaps if you normally wear glasses all day then the corrected vision test reports the result of reading the eye chart with them on? I would expect that the form has to be completed in English throughout, since most documents have to be submitted in English and/or Chinese translation.
