This link should be able to provide most of the information you need.  Now you just need to look for the matching pinyin and write the appropriate 偏旁 in to the box.

 

For example, if you look through that document for jīnzìpáng, you'll see it looks like 钅, which is what you would write in the box.  Note that all of the names of the radicals are descriptive of where they come from or what they mean.  E.g. jīnzìpáng 金字旁 means the 金 character side-radical, because it appears on the side of other characters and is a simplification of which is a derivation of the 金 character.  Another example is 氵which is 三点水 - three drops of water, because it's three little strokes and this radical usually conveys the meaning of liquid/water/wet.

 

All the other radicals will have a similarly descriptive name, so as you're looking them up, you can try to figure out why they have that name.
