Foreigner Physical Examination Form

May I know how should we fill in Foreigner Physical Examination Form, for China scholarship application? I know that we should ask physician or doctors to fill in the form, but I'm unsure, can I find a clinic (either public or private) for the medical checkup? Is it valid?

Sorry to ask, but yeah I'm confusing can I go to a clinic for the examination, since my seniors prefer to go to hospital. And b'cause of the outbreak now, I'm quite not willing to go hospital for the physical examinations.

Which country are you in? 

 

You would normally find a clinic that deals with medical examinations for visa applications.

Malaysia. Hmmm I try to look for it... Is any government hospital or clinic ok? Sorry as this is my first time going overseas ?

It should be done in a government hospital. RPR, VDRL, HBSAG, HIV, Xray tests should be conducted. Make sure you collect the blood test report and xray photo. You are to bring them all to China.
